6th grade parents and guardians:
Every spring the school nurses in conjunction with Transylvania Public Health offer our rising 7th graders the opportunity to receive the two required immunizations for incoming 7th graders, MCV4 and Tdap. Transylvania Public Health prints the permission forms and will provide an on-line option to complete permission forms via survey monkey. All of this information can be found in the permission form and on the Transylvania Public Health
website.
We are planning to send permission forms home to parents/guardians on Thursday, March 16th. On-line options will go live on this date as well. We ask that permission forms are returned by Monday, March 27th so we can determine if vaccine needs to be ordered. We plan to administer MCV and Tdap to those students with parent/guardian permission during the week of April 17th. That same week we will complete MCV4/Tdap campaigns at BMS and RMS, and offer MCV4 at both BHS and RHS to rising Seniors to meet the state requirement.
